Submitting this form is the first step in the nomination process. While submission does not guarantee selection, each nomination is carefully reviewed by our team. If your nominee is chosen, we will contact you directly with next steps.

Windows for a Warrior Eligibility & Consent Information

To be eligible for the Windows for a Warriors program, nominees must reside in Wisconsin and provide documentation verifying military service and discharge status. Please note that submitting a nomination form does not guarantee selection.

By submitting a nomination, you agree to the following:

• Nominee owns their home and lives within the state of Wisconsin.

Information shared on the form may be used publicly in Square Oaks communications, including newsletters, press releases, social media, and other promotional materials.

• Photos or videos submitted to or captured by Square Oaks may be used to promote the Windows for a Warrior program and its impact.
• If selected, please understand that Square Oaks Home Improvements is not responsible for any future maintenance, repairs, or associated costs of the products installed. Square Oaks does not offer additional warranties beyond those provided by the manufacturer.
